    What is a Facelift?

    The disparity between people’s mental image of themselves and the image in the mirror gets more pronounced as they age. This makes them endeavour to improve their facial appearance as they feel young on the inside but look old on the outside.

    Facelift is a wide spread, common facial rejuvenation procedure which subtracts years from a person’s appearance. The idea is to create a well rested youthful appearance. Additional procedures are commonly carried out to the forehead, eyelid and lip areas to further enhance results.

    Facelift can remove excess skin. Tighten the muscle., reposition the under laying soft tissue to remove the excess fat and eliminate the signs of aged appearance such as a sagging, jowls, wrinkling and hanging of excess fat and restore the youthful revitalised look.

    It is not simply a tightening procedure; it is a restoration and redistributing of soft tissue and removal of excess skin. You may need a liposuction to contour the jaw line and the upper neck and under the chin. A smooth jaw line and a nice angle between the neck and chin and a well defined cheek will give a good aesthetically pleasing appearance.

    The double chin can be reduced by liposuction and 'turkey neck' can be reduced by tightening the separated neck muscle.

    A standard facelift is a real surgery, with real benefits. There are various marginal procedures that only correct part of the aging process and may need earlier repletion or surgery to correct other areas soon (like painting part of the wall rather than uniformly restoring ). Every patient being different, so an Individualised approach is necessary.

    The average age for a facelift is in the 50s. In some cases, individuals may want it as young as 40. Factors responsible for aging are genetics, sun exposure, tobacco usage, excess alcohol consumption, stress, and physical illness (such as acne, shrinkage of fat and muscle disorder).

    Commonly soft tissue shrinkage (atrophy) over time can age a face. Gravity contributes to its ageing as well. Other factors that influence ageing are as skin elasticity, bone structure, thinning which may need further procedure like augmentation, peeling and skin care.

    Natural look is top priority where the patient’s look is enhanced without evidence of surgery both during static and dynamic movement such as speaking or smiling. The surgically enhanced area must blend and balance with the rest of the face.

    Facelift is a 3 dimensional, innovative, imaginative sculpting of the face which truly uplifts the patient’s spirit (one with realistic expectations). Facial beauty and harmony consists of combination of contour, shape and 3 dimensional volume restorations in the entire face from forehead to collarbone. The volume restoration is the critical goal for rejuvenation. The individuals should describe well the goals, concerns and priorities to the surgeon during consultation. Facial asymmetry is very common and should be noted well before the surgery.

    Maintaining the balance between the skin, soft tissue and gives a more pleasing result. The facelift gives structural support and other combination procedures such as laser peel ,refines and enhances quality of skin fillers and implant augment soft tissue, botox reduces expression lines.

    Usually, face lift results last for 7 to 10 years.

    Types of Facelift

    Skin only facelift : separation of top layer of skin from the fat and muscle, pull, redrape and remove teh excess

    SMAS Facelift : Deep layer of sheet of muscle and fiburous  tissue separated, pulled , tightened and repositioned. It holds the tension and releases the tension of the skin. It gives a finely chiselled, elegant and sophisticated look with better definition. The vector of the pull of the skin and the deep layer are in different plane. This deep layer acts like a handle to lift the saggy tissue. This technique is a multiple plane technique.

    Short scar or S shaped incision lift : Mainly placed in front of the face. This type is usually effective for the younger patient with quicker recovery, less bruising and swelling, mainly effective for the smile fold and jowl. It allows people to pull the hair back without fear of revealing scar in the back of the ear.

    Endoscopic facelift : Is performed using a camera through small scars behind the hairline. Ideal for people with droopy brow, saggy cheek, fold around the mouth with less excess skin and mostly droopiness of teh soft tissue. Mostly people in their late thirties and early forties.  The advantage is less scarring, quick recovery, less risk of complication.

    Secondary facelift : Variable technique depending on a number of factors due to previous surgery, lifestyle and anatomy. The extent varies from a touch-up to a full surgery. The existing scars limit teh planning in incision and approach.
